Neuroscience Services

At Methodist Hospitals, a highly qualified team of neurosurgeons, neurologists, neuroradiologists, internists, emergency medicine physicians, nurses, MS certified nurses, social workers, and support staff ensures the Neuroscience Institute provides patients with superior care and award-winning programs. The recent honor of being ranked 45th in the nation for Neurology and Neurosurgery in U.S. News and World Reports’ 2008 publication of America’s Best Hospitals further reinforces the commitment of the physicians and staff. 

Methodist Hospitals Neuroscience Services provides diagnosis and treatment for diseases and injuries of the central nervous system, including the brain and spinal cord.

Delicate neurosurgeries and sophisticated spinal procedures are performed using the latest technology and facilities. Chronic Pain Services are available to assist a patient in dealing with and controlling pain.

The option of Radiosurgery, a non-surgical treatment for select brain tumors, is the first of its kind in Northwest Indiana.

  • Brain Attack is a program offered to community groups. Included are blood pressure screenings, stroke profiles and risk factors presented by an R.N.
  • The Acute Stroke Program is initiated for "Brain Attack" patients arriving at the emergency room within three hours of the first symptoms.

Support groups include Head Injury, Multiple Sclerosis, Parkinson's Disease and Seizure.

Our programs include:

Multiple Sclerosis Program

The Mulitple Sclerosis Program effectively manages clients with multiple sclerosis utilizing a multi-team, holistic approach, concentrated at one center in order to increase client well being and productivity.

Each new patient will receive an MS folder on their first visit, the following is the pathway for appointments:

1st office visit:

  • Detailed history taken by RN
  • Physical assessment performed by RN
  • Insurance and benefits check provided by social worker
  • Provide patient with MS diary and instruct on purpose and use
  • Provide support group information to patient and family
  • Initial evaluation by physician
  • Lab work drawn on-site if needed
  • IV treatments or injections given on-site if needed
  • Needs identified (physical, emotional, social, etc.)
  • Plans of care designed (medical and nursing)
  • Teaching initiated (medications, treatments, therapies, diagnostics,etc.)
  • Documentation of visit in SOAPIE format in chart
  • Follow-up appointment made for one month or as needed

2nd office visit:

  • Updated history taken by RN
  • Evaluation of previous plan of care by RN and/or physician
  • Review MS diary and interpret findings
  • Review of all previously ordered labs and diagnostics
  • Physical examination by RN
  • Perform IV treatments, injections, and/or labs on-site if needed
  • Register patient with the National MS Society (with consent)
  • Assess frequency of follow-up visits
  • Identify teaching needs and provide teaching session
  • Physical examination by physician
  • Documentation of visit in SOAPIE format in chart

Follow-up visits:

  • Comprehensive assessment of plan of care
  • Schedule annual exam (Bladder scan, labs, vision screen, and exam)
  • Address chief complaint and report to physician
  • Yearly physical by physician
  • Document progress in SOAPIE format
  • Identify and implement current teaching plan

The Comprehensive Stroke Program

Stroke is the third leading cause of death in the United States and a major cause of chronic disability. Methodist Hospitals Neuroscience Services is actively involved in the comprehensive care of patients with stroke and related disorders. The Comprehensive Stroke Program was developed to coordinate three major areas relating to stroke: patient care, education, and research. A major focus of the Comprehensive Stroke Program is stroke prevention. The Comprehensive Stroke Program brings together the combined expertise of physicians in a number of medical specialties, as well as medical personnel involved in education, rehabilitation, therapy, nutrition and social service.

The Brain Tumor Program
(a Multi-team approach)

The Brain Tumor Program manages clients with brain tumor(s) utilizing a multi-team, holistic approach at an outpatient center in order to increase client well being and productivity.

Each new patient will receive an information packet at the first appointment or visit (guide to stress management brochure, NIH Brain and Spinal Cord Tumors, American Brain Tumor Association pamphlet). The following is the pathway for appointments:

 

1st Contact — Step 1 (initial contact/plan with Nurse/MD)

  • RN/NP discussion of brain tumor (types/causes/signs and symptoms) (1/2-1 hour)
  • RN/NP interviews patient (history/ROS) (1/2 hour)
    Physician or Nurse Practitioner sees the patient (exam/diagnosis/plan/tests)
  • RN/NP reviews the plan/medications/tests with patient and family
  • RN/NP will document in chart
  • MD examines patient, devises plan of care, discusses medications, test, test results, etc. with patient and family. Consults with primary physician/neurosurgeon (continues this feedback throughout process)
  • Brain tumor team reviews at next meeting

2nd Contact — Step 2 (follow up contact with MD/NP)

Pre-Surgery:

• RN/NP assesses patient in hospital, reinforces teaching to patient and family, identifies needs of patient and family
• MD/Neurosurgeon sees patient/family

Post-Surgery:

• RN/NP assesses patient for follow up needs
• RN review plan of care/medications/discharge instructions, etc. with patient and family
• Follow up appointment scheduled for two weeks post discharge
• MD/Neurosurgeon sees patient/family

3rd Contact — Step 3 (follow up appointment with MD/NP)

• RN/NP reviews current treatment, evaluates plan of care effectiveness
• RN/NP assesses patient, identifies needs of patient and family, teaching reinforced
• RN/NP refers to Social Services/support group
• RN/NP schedules follow up appointment for one month
• MD performs exam, re-evaluates plan of care, discusses medication, further tests, etc.

Follow Up Appointments — Step 4 (follow up with MD/NP/RN)

• RN/NP evaluates plan of care effectiveness
• RN/NP performs assessment, teaching reinforced, identifies needs of patient and family
• RN/NP schedules follow up appointment for three months, six months and one year and monitors patient and family needs through monthly phone calls the first three months, then quarterly phone calls made to the home for the first year.
• MD examines and evaluates patient, orders further testing, medications, etc. as needed.

The Chronic Pain Program

Anesthesiology provides both inpatient and outpatient consultation of Pain Management Services for the treatment of acute and chronic pain.

Methodist Hospitals Neuroscience Service provides outpatient care specialists for chronic and intractable pain, providing evaluation and treatment of both medical and surgical options.

The Headache Program

To effectively manage clients with different types of headaches with a multi-team, holistic approach, concentrated at one center in order to increase client well-being and productivity.

Procedure: Each new patient will receive an information packet from NSIC before coming to their first appointment. (headache brochure/NSIC brochure/headache diary with instructions to fill out before appointment), the following is the pathway for appointments:

1st Appointment

STEP 1 (initial contact/plan with MD)

  • RN discussion of headaches    (types/causes/triggers/etc...) (1/2-1 hour)
  • RN interviews patient (history/ROS)
       (1/2 hour)
  • Physician sees the patient (exam/diagnosis/plan/tests)
  • RN reviews the plan/medication/test with patient and family/referral for biofeedback
  • RN will document in chart

2nd Appointment

STEP 2 (follow up contact with MD or NP)

  • Approximately 3-4 weeks
  • Follow-up consultation with Nurse Practitioner or Physician (exam/plan)
  • review progress/medications
  • review diary
  • review lifestyle
  • refer for follow-up appointment with MD if needed

 

3rd Appointment
STEP 3 (follow up contact with MD or NP)
Physician or Nurse Practitioner sees patient for follow up RN review plan/medication etc...

Follow-up appointment schedules:
STEP 4
IE: if on Preventives--every 6 months
IE: if on Abortives--every 3 months
IE: stable patient-yearly

At any point, patient can make follow-up appointment with the NP

• review medication
• review lifestyle
• re-examine plan
• make changes in plan
• refer to MD as needed
• refer to dietician/psychologist as needed
  NP will consult with MD on a regular basis
  regarding patient's plan and progress
