HEADACHE PROGRAM

Purpose:

To effectively manage clients with different types of headaches with a multi-team, holistic approach, concentrated at one center in order to increase client well-being and productivity. Procedure: Each new patient will receive an information packet from NSIC before coming to their first appointment. (headache brochure/NSIC brochure/headache diary with instructions to fill out before appointment), the following is the pathway for appointments:

1st Appointment
STEP 1 (initial contact/plan with MD)
• RN discussion of headaches    (types/causes/triggers/etc...) (1/2-1 hour)
• RN interviews patient (history/ROS)
   (1/2 hour)
   Physician sees the patient    (exam/diagnosis/plan/tests)
• RN reviews the plan/medication/test with
   patient and family/referral for biofeedback
• RN will document in chart

2nd Appointment
STEP 2 (follow up contact with MD or NP)

Approximately 3-4 weeks
Follow-up consultation with Nurse Practitioner or Physician (exam/plan)

• review progress/medications
• review diary
• review lifestyle
• refer for follow-up appointment with MD
  if needed


3rd Appointment
STEP 3 (follow up contact with MD or NP)
Physician or Nurse Practitioner sees patient for follow up RN review plan/medication etc...

Follow-up appointment schedules:
STEP 4
IE: if on Preventives--every 6 months
IE: if on Abortives--every 3 months
IE: stable patient-yearly

At any point, patient can make follow-up appointment with the NP

• review medication
• review lifestyle
• re-examine plan
• make changes in plan
• refer to MD as needed
• refer to dietician/psychologist as needed
  NP will consult with MD on a regular basis
  regarding patient's plan and progress
