THE BRAIN TUMOR PROGRAM

Purpose:
To effectively manage clients with brain tumor(s) utilizing a multi-team, holistic approach at an outpatient center in order to increase client well being and productivity.

Procedure:
Each new patient will receive an information packet from NSIC at the first appointment or visit (guide to stress management/NSIC brochure, NIH Brain and Spinal Cord Tumors, American Brain Tumor Association pamphlet). The following is the pathway for appointments:

1st Contact — Step 1 (initial contact/plan with Nurse/MD)

• RN/NP discussion of brain tumor (types/causes/signs and symptoms) (1/2-1 hour)
• RN/NP interviews patient (history/ROS) (1/2 hour)
Physician or Nurse Practitioner sees the patient (exam/diagnosis/plan/tests)
• RN/NP reviews the plan/medications/tests with patient and family
• RN/NP will document in chart
MD examines patient, devises plan of care, discusses medications, test, test results, etc. with patient and family. Consults with primary physician/neurosurgeon (continues this feedback throughout process)
• Brain tumor team reviews at next meeting

2nd Contact — Step 2 (follow up contact with MD/NP)

Pre-Surgery:

• RN/NP assesses patient in hospital, reinforces teaching to patient and family, identifies needs of patient and family
• MD/Neurosurgeon sees patient/family

Post-Surgery:

• RN/NP assesses patient for follow up needs
• RN review plan of care/medications/discharge instructions, etc. with patient and family
• Follow up appointment scheduled for two weeks post discharge
• MD/Neurosurgeon sees patient/family

3rd Contact — Step 3 (follow up appointment with MD/NP)

• RN/NP reviews current treatment, evaluates plan of care effectiveness
• RN/NP assesses patient, identifies needs of patient and family, teaching reinforced
• RN/NP refers to Social Services/support group
• RN/NP schedules follow up appointment for one month
• MD performs exam, re-evaluates plan of care, discusses medication, further tests, etc.

Follow Up Appointments — Step 4 (follow up with MD/NP/RN)

• RN/NP evaluates plan of care effectiveness
• RN/NP performs assessment, teaching reinforced, identifies needs of patient and family
• RN/NP schedules follow up appointment for three months, six months and one year and monitors patient and family needs through monthly phone calls the first three months, then quarterly phone calls made to the home for the first year.
• MD examines and evaluates patient, orders further testing, medications, etc. as needed.
